Brain fog vs ADHD: how to tell what you're dealing with

Fatigue, poor focus, and mental sludge can come from ADHD—or from sleep, iron, thyroid, and other treatable causes.

"Brain fog" is not a formal diagnosis. It is a description—thinking feels slow, words are hard to find, you re-read the same paragraph, you forget why you walked into a room. ADHD can feel like that. So can iron deficiency, thyroid dysfunction, poor sleep, post-viral fatigue, and depression.

Clues that point toward ADHD

  • Long-standing pattern since childhood or adolescence, not only after an illness
  • Inconsistent focus—hyperfocus sometimes, blankness other times
  • Executive struggles: planning, prioritising, finishing
  • Improvement with structure and deadlines; collapse when structure disappears

Clues that suggest looking beyond ADHD first

  • Sudden onset after fever, COVID, or major sleep disruption
  • Heavy periods, hair loss, or exhaustion with normal-looking routine labs (still worth reviewing with a physician)
  • Prominent low mood or loss of interest alongside fog
